Patients who are new to CSSC will need to complete two steps by logging into the ModMed patient portal, prior to their first visit: (1) complete all office consent forms (2) complete your medical history, including medications, and allergies

Established patients may message the practice regarding medical questions or request prescription refills through the same patient portal.
